Each pupil shall receive a comprehensive health examination upon original entry, while in sixth grade and in eleventh grade Per policy 209.
Each pupil shall receive a comprehensive dental examination upon original entry, while in third grade and in seventh grade per policy 209.
No “over-the-counter” medication may be administered by any school district employee without specific written instructions from a licensed physician/dentist, with the following exceptions, which are available in the Health Room of each school: antacid tablets, cough drops, throat lozenges and acetaminophen (secondary only, at the discretion of the school nurse, with written with written permission from parent/guardian--renewed yearly Per Policy 208.
It is required by the North Hills School District that the attending physician complete this form for any medication to be given during school hours. This form is only good for the school year in which it is signed. All medication must be sent to school in the original container bearing the medication name and, if prescription, the current dose. Please return the signed form to your child’s school nurse.

Two (2) weeks prior to an absence for travel, an Educational Field Trip Request Form must be completed and returned to the school office for approval. Students are expected to complete assigned class work during absences for travel or vacation. A maximum of five (5) cumulative days in any given school year may be considered excused upon completion of the Educational Field Trip Request Form.
All students who will be participating in school-sponsored athletics must have a physical prior to the start of the sports season. The comprehensive physical exam is valid for one year and must be dated June 1 (of the current year), or later.
Multi-sport student athletes must have a completed Re-Certification form on file in the Athletics Office prior to beginning a new sports season within the same school year.
This form must be completed and signed by a physician, if your child has a disability that requires special dietary accommodations or substitutions.
