Back to School: Forms can now be completed in PowerSchool

There are three forms that parents should complete in PowerSchool in PowerSchool prior to the start of the school year.

The "Forms" section is accessible from the left menu options when logged into PowerSchool on a web browser. You must use a web browser on a computer the first time you access forms. You will see a yellow bar at the top of your screen that reads, “Your preferences haven’t been configured.” This yellow bar will prohibit you from seeing the forms on a browser when accessed on a mobile device. Please use a computer web browser. To configure your preferences, simply click the gear option. Doing so will enable you to see forms in the future via a web browser on a mobile device.

On the first day of school, students will be bringing home a paper emergency card that will also need to be filled out and returned to your child’s school.

Please follow the instructions below to complete the forms listed above online:

  • Student Address and Contacts
  • Emergency Contacts
  • Permissions/Agreements

Any email from FinalForms on behalf of your child’s school should be disregarded.

Please follow the instructions below to complete your child's forms:

  1. Navigate to PowerSchool and log in.
    • If you have not already set up your PowerSchool account, please contact your student’s school as you will need their Access ID, Access Password and their name as it appears in PowerSchool.
    • Steps on creating an account can be found here.
  2. Once your child’s schedule is available, you will also have access to complete the Forms section. Select "Forms" from the left menu items. Remember: You must be logged into PowerSchool from a Web browser. Forms are not accessible from the PowerSchool app.
  3. Select each form to review and update your child’s information. Required fields are marked with an asterisk (*) and an electronic signature is required for each form.
    • Please note the legend box explaining the icons. School staff will review each submission. Once approved, information will automatically update in PowerSchool. If a form is rejected, you will have an opportunity to make corrections.
  4. Under R5. Permissions/Agreements and in accordance with our iPad Use and Loan agreement, each iPad must be registered with One2One Risk Solutions. You will have the option to purchase or waive iPad insurance each school year, but each iPad must be registered every year.
    • NHSD will waive the $35 insurance premium for families who qualify for the Free and Reduced Lunch Program. These families must still register the device and are responsible for any deductibles should a claim be submitted.
