COVID-19 vaccination clinic for students ages 5-11 set for Nov. 18, Dec. 9

NHSD is hosting a COVID-19 vaccination clinic for students ages 5 to 11 at North Hills High School.

The first dose of the Pfizer vaccine as approved for children ages 5-11 will be administered by Giant Eagle pharmacists at North Hills High School on Thursday, Nov. 18 from 4 p.m. to 8 p.m. The second dose will be given on Thursday, Dec. 9 during the same time period.

Parents/guardians of NHSD students interested in receiving the vaccine must complete this registration form. Spots are limited so please sign up as soon as possible. This form will close on Thursday, Nov. 11 at 8 a.m. or when all spots are filled. One form must be completed for each student. You must fill out this form regardless if you filled out the interest survey.

At this time, this clinic is open to students ages 5-11 only.

Once registration closes, an email will be sent to those who registered with a link to sign up for an appointment time. Appointments will be offered between 4 p.m. and 8 p.m. The time your child receives their shot on Nov. 18 will be the same time they receive their second dose on Dec. 9.

Please print and complete the form linked here (and below) and bring it with you on Nov. 18. Please also bring the student’s health insurance and prescription cards, and student’s ID, if applicable.

Please note:

  • Students must be age 5-11 on Nov. 18 to qualify for this clinic.
  • This clinic is voluntary without any recommendation, direction or coercion from anyone at NHSD.
  • This clinic will be facilitated by Giant Eagle and its pharmacists.
  • NHSD is not liable for any injury, loss or damage that may occur following vaccination.
  • Those scheduled for an appointment should NOT report if they or someone in their household is exhibiting any sign or symptom of COVID-19, or if they were identified as a close contact of someone diagnosed with the virus.
