Immunizations & Exams

Immunizations & Exams for Enrollment

Per Pennsylvania Department of Health and Allegheny County Health Department, North Hills School District immunization requirements for enrollment are as follows:  

  • 4 doses of tetanus, diphtheria, and acellular pertussis* (1 dose on or after the fourth birthday and usually given as DTP or DTaP or if medically advisable, DT or Td)
  • 4 doses of polio (4th dose on or after fourth birthday and at least 6 months after previous dose given. A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose.)
  • 2 doses of measles, mumps, rubella (Usually given as MMR)
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) or evidence of immunity

Immunizations & Exams by Grade Level

The exams and/or immunizations are required for students entering grades 3, 6, 7, 11 and 12. Completed exam forms and updated immunization records should be submitted to the student’s school nurse prior to the start of the school year.

Students entering Grade 3

  • Dental exam dated August 1 of the previous year or after.

Students entering Grade 6

Students entering Grade 7

  • Dental exam dated August 1 of the previous year or after.
  • 1 dose of tetanus/diphtheria/pertussis (Tdap) after student's 11th birthday
  • 1 dose of meningitis vaccine (MCV4) after student's 11th birthday

Students entering Grade 11

Students entering Grade 12

  • Second dose of MCV4 after student's 16th birthday

Completed exam forms and updated immunization records can be dropped off at the school or emailed to the appropriate school nurse as listed here.

The school district dentist and physician can complete the exams above if preferred.

Immunization records must include each dose of vaccine and the full date (month/day/year) it was administered.

If a student has not received all of the required vaccines, the student may be provisionally admitted to school only if evidence of the administration of at least one dose of each vaccine is provided to the school and the parent/guardian’s plan for completion of the required immunizations is made part of the child's health record. Immunization requirements should be completed within eight months of the date of provisional admission to school.

Students not in compliance with the immunization requirements above must be excluded from the start of the school year until the student can verify the immunizations were received, has a scheduled plan signed by the physician or obtains an exemption signed according to the provisions of the law. Please note that exempted students may be removed from school in the event of an outbreak.
