Physical Form

Per policy 209 ~ Each pupil shall receive a comprehensive health examination upon original entry, while in sixth grade and in eleventh grade.
Immunization Update Form

All students in kindergarten through grade 12 will be required to have a second dose of the varicella (chickenpox) vaccine and all students in grades 7 through 12 will be required to have a Tdap vaccine as well as a meningitis vaccine.
Dental Form

Per policy 209 ~ Each pupil shall receive a comprehensive dental examination upon original entry, while in third grade and in seventh grade.
Facility Request Form

This form is used to apply for a building or field use permit. Please follow the directions carefully and allow two weeks for processing.
Medication Forms

Per Policy 208 ~ No "over-the-counter" medication may be administered by any school district employee without specific written instructions from a licensed physician/dentist, with the following exceptions, which are available in the Health Room of each school: Antacid tablets, cough drops, throat lozenges, Acetaminophen (secondary only and at the discretion of the school nurse, with written permission from parent/guardian - renewed yearly.) Prescribed medication may be administered by school personnel only with the specific written instruction of the attending physician/dentist and with permission of the parent/guardian.
2010-11 After-School Care: Registration Form

This service, which is provided by the North Boroughs YMCA, serves students in kindergarten through grade 6. The YMCA School Age Child Care Programs for the North Hills School District will be housed in the Highcliff, McIntyre and West View Elementary Schools during the 2010-2011 school year.
Elementary Registration Packet

This packet includes registration forms needed for all students entering grades 1 through 6.
Secondary Registration Packet

This file includes all forms necessary to register students in North Hills Junior High School (grades 7-9) and North Hills Senior High School (grades 10-12). Parents/guardians will also be asked to complete an emergency card, which can be obtained from the central registration clerk or from the school nurse.
Student Educational Field Trip Form

Two (2) weeks prior to an absence for travel, an Educational Field Trip Request Form must be completed and returned to the school office for approval. Students are expected to complete assigned class work during absences for travel or vacation. A maximum of five (5) cumulative days in any given school year may be considered excused upon completion of the Educational Field Trip Request Form.
