Health Services Reminders

Physical Exams: Students entering grades six and eleven require a comprehensive health examination. Download the physical form

Immunizations: The Allegheny County Health Department School Immunization Requirements are as follows…

Immunization regulations for all students K – 12

o 4 doses of tetanus (1 dose after the 4^th birthday)

o 4 doses of diphtheria (1 dose after the 4^th birthday)

o 3 doses of polio

o 2 doses of measles

o 2 doses of mumps

o 1 dose of rubella

o 3 doses of hepatitis

o 2 doses of varicella or written statement from physician/designee indicating month and year of disease or serologic proof of immunity

Grades 7-12 (in addition to the above)

o 1 dose of tetanus/diphtheria/pertussis (Tdap)

o 1 dose of meningitis vaccine (MCV4)

Dental Exams: Students entering grades three and seven require a comprehensive dental examination. Download the dental form
Sports Physicals: All students who will be participating in school-sponsored athletics must have a physical prior to the start of the sports season. The comprehensive physical exam is valid for one year. For more information, visit the athletics website: www.nhsports.org
