Grades 7, 8 and 9 

Prior to Grade 7, 8 and 9 students receiving iPads, parents/guardians must review the Project Connect Student/Parent iPad User Handbook below. This handbook provides an overview of the various components of Project Connect.  

After reviewing the handbook, you must review or submit the following paperwork in one of three ways:

  • NHSD Student iPad Use/Loan Agreement
  • NHSD Student/Parent Responsible Use Agreement
  • NHSD Network Acceptable Use Policy
  • NHSD Procedure on Tracking Mobile Devices
  • Parents/guardians also should review the Project Connect iPad Insurance and Coverage information (included in the Student/Parent iPad Handbook and also posted individually below) that notes repair and replacement costs. Parents/guardians must enroll the student device in the website detailed on the insurance form and choose to either accept or decline the insurance option prior to the issuance of the student device. The window to accept/decline insurance is July 1 to August 1, 2017!  To complete this task, you will need your student's identification number which was printed on your student's 2016-17 second semester report card.

To submit each of these forms, Grade 7, 8 and 9 parents may:

  1. Submit the forms electronically by clicking here beginning July 1, 2017.
  2. Bring the forms to an orientation or curriculum night event. Click here for dates. Please note Grade 9 Orientation is being offered several times this summer. We encourage parents make every effort to attend one of the sessions for the grade-level of their student.
  3. Drop the forms off at North Hills High School/North Hills Middle School beginning July 1, 2017.
 
 
Grade 6 
 
Parents/guardians are asked to bring the four signed documents noted below and included in the Project Connect Student/Parent iPad Handbook and also posted individually to the Project Connect Grade 6 Parent Meeting on August 28 at 6:30 p.m. in the middle school auditorium. This will expedite the iPad distribution process.

  4. NHSD Procedure on Tracking Mobile Devices

In the event your student’s iPad is in need of service or damaged, the student and parent must complete this iPad Damage Report Form which also is available in the principal’s office. The completed form must be submitted to the building principal’s office with the iPad in need of service or repair as soon as possible. A loaner device may be issued during services or repairs.
This form must be completed by parents who were not able to attend the Grade 6 Project Connect Parent Meeting on August 28, 2017. Please watch the video of the meeting's presentation below. After viewing, turn in this form along with the other required paperwork to your student's building principal.

iPad Insurance and Coverage Information
Parent/guardians are asked to review the insurance and coverage information detailed in this document. Insurance must be accepted or declined via an online process before a student device will be issued. The window to accept or decline insurance for Grades 7, 8 & 9 is July 1 to August 1, 2017.
